The data collection process collects conditions that are found on the clinical record screen. Chronic conditions are flagged as true for a patient if the condition is recorded in the patient's clinical record. For details on the list of active and inactive conditions please refer to the relevant document here.
Conditions are mapped using ICPC Code and ICPC Term Code to determine if a condition is to be included.


CAUTION: This list of ICPC codes needs to be reviewed after each release of ICPC2-Plus to include new codes.
