This report will provide practices with a list of patients that demonstrate The Pen CS CAT4 software product provides easy access to reporting relating to patients that have osteoporosis risk factors. The report will list all patients that have no coded diagnosis of osteoporosis, but meet risk factors relating to osteoporosisthe disease.

The report will apply the following mandatory filter requirements when populating the list of patients.

• Patients that have experienced a fracture since the age of 50

The report can be accessed through the Dashboard for by licensed users only . The by using the 'Programs' icon in the dashboard. The dashboard programs icon will be active for users participating in any particular program and greyed out for all others.

Clicking on the program icon will open CAT4 and show the relevant report called REFRAME Osteoporosis.
