The data collection process collects medications that are found in the patients current medication list on the Summary Views Patient View > Medications tab > Current Medications. CAT4 does not make any decisions about whether a medication should actually have been removed from the patient's current medication list. The GP is responsible for making sure the list of medications is accurate and up to date.
