The table below lists the data items that make up the Diabetes Cycle of Care. It combines data items already provided in the preceding sections and is provided for the convenience of having these data items listed together. All pathology items are listed for reference with their manual data entry option. The default for pathology is HL7 download and manual entry should not be required but is an option if needed.
