Risk Stratification in CAT assists the General Practice to practise preventive care, by highlighting potentially preventable hospitalisation, aligned with RACGP’s ‘Standards for General Practice’ Criterion C4.1. 

Through CAT, practices can

  • Lead towards better care, pre-planning for follow-ups, and improve patient outcome.
