This report checks the following items for active (3 visits in the last 2 years) patients in line with the Cleansing View reports in CAT4:

•Weight
•Smoking
•Alcohol
As the data in PAT CAT is de-identified no reports on missing address, contact details or other personal information is available. The 'View Report' button will display the detailed numbers and percentages for each indicator and clinic.
Please note that the target population is defined as any patient with at least one of the items reported on missing from their record.
