The data collection process collects conditions that are found on the Best Pratice 'Past History' screen in both the active and inactive parts of the screen. Chronic conditions are flagged as true for a patient if the condition is marked as either active or inactive.
Other conditions, which are regarded as being recoverable, are flagged as true if they are marked as active only. In the list below active only conditions are noted – all others are reported regardless of their status in the clinical system.
