Recipe Name:

Patients with a chronic disease diagnosis of Diabetes, Asthma, COPD, CHD, Heart Failure, Stroke, Renal Impairment and CKD

Rationale:

Patients with chronic diseases are at high risk for Covid-19. This recipe will show you how to assess the numbers of these patients in your catchment

Target:

Patients with diabetes, asthma, COPD, CHD, heart failure, stroke, renal impairment and/or CKD

Recipe Limitations:

 

CAT Starting Point:

  1. CAT Open - CAT4 view (all reports) loaded
  2. Population Extract Loaded and Extract Pane "Hidden"

Report Steps

  • Click on the 'Disease' report tab and select the 'Disease Prevalence' report

The adjusted column in the report will show the numbers of patients in your catchment for each of the chronic conditions. You can also look at individual practices to identify practices with a higher than average count of patients with chronic conditions.
Other filters can be combined with this recipe if required, such as 'active patients' or other criteria.
