Find all patients aged 75 and older, with existing chronic conditions which are associated with a higher risk of death

Rationale:

Patients aged over 75 with multiple chronic health conditions are at higher risk of death, with over 80 % of deaths occurring in the 75+ age bracket in Australia ¹.

The leading cause of death for all older Australians is coronary heart disease, followed by dementia and Alzheimer disease, cerebrovascular disease, and then chronic obstructive pulmonary disease (respiratory condition) ¹.

Patients living with chronic, life limiting conditions are at higher risk of not having their palliative care needs identified, compared to those with malignant conditions ².

This recipe assists practitioners to identify patients with potential palliative care needs.

Note: as stroke / cerebrovascular disease is often identified only after a stroke event, renal conditions are included instead ³.
