Covid-19: Patients with specific chronic diseases

Recipe Name:

Patients with a chronic disease diagnosis of Diabetes, Asthma, COPD, CHD, Heart Failure, Stroke, Renal Impairment and CKDchronic conditions

Rationale:

Patients with chronic diseases are at high risk for Covid-19. This recipe will show you how to assess the numbers of these patients in your catchment

Target:

Patients with diabetes, asthma, COPD, CHD, heart failure, stroke, renal impairment and/or CKDa current diagnosis of a chronic condition

Recipe Limitations: 

 Not all chronic conditions are reported in PAT CAT

CAT Starting Point:

  1. PAT CAT Open - CAT4 view (all reports) loaded
  2. Population Extract Loaded and Extract Pane "Hidden"

The adjusted column in the report will show the numbers of patients in your catchment for each of the chronic conditions. You can also look at individual practices to identify practices with a higher than average count of patients with chronic conditions.
