 Recipe Name:

Find all patients currently residing in a RACF who are likely to benefit from a palliative care approach.

Rationale:

Patients living in RACFs are older, frail, have poor functional status, multiple comorbidities and compromised immune systems 1. This means that a palliative approach is often appropriate for most residents 2. Palliative care is recognised as core business for RACFs3.

RACF residents receive less specialist palliative care support than those living at home and therefore are at greater risk of not having their palliative care needs identified and addressed4.

This recipe assists practitioners, who are VMOs and care for patients residing in RACFs, to identify patients with potential palliative care needs.

Target:

Patients residing in a RACF, based on MBS items claimed that are only available to such patients.

Recipe Limitations:

  1. CAT Open - CAT4 view (all reports) loaded
  2. Population Extract Loaded and Extract Pane “Hidden”

Filter Steps

On  the MBS Attendance filter tab, select the RACF category. If you want to only filter for active patients as well, select the 'Active' box on the general filter tab.



Now all filter criteria have been set, please click on 'Recalculate' to apply the filter:


To see all patients with any RACF related MBS items, click on the 'View Population' icon:

This will show a report with all patients meeting your criteria and the last two columns will show their last visit and their assigned provider.



From this report you can export the patient list to Excel or send voice or text messages directly from CAT4. To export a list, click on the disk icon and choose your preferred file format:
